Diabetes and metabolic care with a doctor who has time for you
Dr. Kathryn Shoemaker, a board-certified internist, works with you on blood sugar, weight, blood pressure and cholesterol together. Your labs are drawn right in our office, you're seen the same or next day, and you can call or text her between visits.
Board-certified internist · Labs drawn in our office · In-office body-composition scans
In person or virtual, with no commitment. Our team will call you to answer questions and find a time.
In-office body composition
See what the scale can't
Your weight is one number. Our in-office InBody body-composition scan shows what it's made of, in about a minute.
Members can repeat the scan as often as they like, so you and Dr. Shoemaker can see your plan working, not just watch a number on the scale.
Continuous glucose monitoring
A small sensor you wear on your arm shows how your blood sugar responds to meals, activity, sleep and stress, around the clock. Dr. Shoemaker reviews the readings with you and uses them to fine-tune your plan, whether you're working to reverse prediabetes or managing diabetes that needs insulin.
How it works
How we work on your metabolic health
One doctor for the whole picture
Blood sugar, weight, blood pressure, cholesterol and heart risk move together. A board-certified internist is trained to treat them together, and as your concierge doctor, Dr. Shoemaker has the time to.
- Your heart, checked too. In-office EKG and ultrasound, and advanced heart screening (calcium score, ApoB, Lp(a)) when it fits your risk.
- Labs in our office. No lines at an outside lab; repeat checks take minutes.
- Visits as long as you need, typically 30 to 60 minutes.
- Coordination with an endocrinologist, dietitian or other specialists when you need one.
Part of your concierge membership
Metabolic care is part of membership at Three Rivers Concierge Medicine. The doctor working on your blood sugar is your primary care doctor for everything else, too.
$325 a month, billed annually.

Insurance still matters. Lab processing, imaging, prescriptions, specialist visits and hospital care go through your insurance as usual, and you can keep using your HSA or FSA for that care.

Frequently Asked Questions
Can prediabetes be reversed?
Often, yes. For many people, changes to nutrition, activity, sleep and weight bring blood sugar back into a healthy range. Dr. Shoemaker builds that plan with you and tracks your numbers so you can see it working.
Do you manage diabetes that needs insulin?
Yes. Dr. Shoemaker manages insulin-dependent diabetes, as well as prediabetes and type 2 diabetes treated without insulin. With labs drawn here, continuous glucose monitoring and a doctor you can call or text, your treatment is adjusted as your numbers change.
Do you prescribe diabetes and weight-loss medication?
Yes, when it's right for you after a full evaluation, with labs and follow-up handled here.
What does the body-composition scan show?
Muscle mass, body fat percentage, visceral fat and hydration, measured in our office in about a minute. Members can repeat it as often as they like to track progress beyond the scale.
Do I still need an endocrinologist?
Most people with prediabetes or type 2 diabetes are cared for well in primary care. If you need an endocrinologist, Dr. Shoemaker refers you and coordinates with them.
Do I need to be a member?
The meet & greet is free and doesn't require membership. Ongoing metabolic care is part of membership.
How much does it cost, and does insurance cover it?
Membership is $325 a month, billed annually. The membership fee isn't billed to insurance. Your insurance still covers lab processing, prescriptions and specialist care.
